Ulthera or Thermage: which should you choose?

One of the most common consultation questions is: “Should I choose Ulthera or Thermage?” Both are nonsurgical lifting and tightening treatments, but they act at different depths and address different concerns. Understanding whether your concern is drooping tissue or loose skin makes the choice clearer.

Different treatment depths

Ulthera uses focused ultrasound (HIFU). Energy reaches approximately 4.5mm deep into the SMAS layer, the same layer addressed in a surgical facelift, as well as the deeper dermis. It addresses sagging caused by weakened support structures.

Thermage FLX uses fourth-generation monopolar radiofrequency to heat the dermis and superficial subcutaneous layers, encouraging collagen contraction and gradual renewal. It is better suited to skin laxity, fine lines and texture.

Both treatments use disposable tips or treatment heads, which account for a substantial part of the cost. NELL uses genuine consumables supplied directly by the importer. Can they be combined?

Yes, and this is common. As they act at different depths, Ulthera lifting and Thermage tightening are often combined when sagging and laxity occur together. Your skin condition and treatment plan determine whether they are performed on the same day or separately, and in which order. The clinic director decides after consultation. Juvelook or Rejuran may also be added when volume or skin quality needs attention.

Before your consultation

  • Tell your doctor about treatments in the past six months, especially fillers and thread lifting
  • Arrive without makeup or be ready to remove it for a skin assessment
  • Let us know in advance about pregnancy, breastfeeding or metal implants
  • Bring photos of the concerns that matter most to you; they can be clearer than a verbal description
